individual respite providers in our database.
Individuals may post their information on the Oklahoma Parent Network
Provider Registry at www.okparentnetwork.org
Agencies that provide respite care
may be listed in our database.
Agency
providers will need to fill out the Respite Provider
Application (in
pdf format) and return to us at the address listed on
the application. Download

Once listed in the database, agency providers are responsible for notifying OASIS of any address or telephone changes or changes in their service provision.
